Orders not accompanied by the signed Authorization (way down near the bottom of this webpage) cannot be shipped. It's OK to delete pages from the middle part of this form that contain only video titles you aren't ordering. Save a tree, but don't forget to include the important parts at the end!
Checks/money orders must be in U.S. Funds, made payable to: The Collector. We cannot accept funds drawn on banks outside the USA.
